Bemiparin versus enoxaparin in the prevention of venous thromboembolism among intensive care unit patients
Conclusion: Bemiparin was superior to enoxaparin as a prophylactic anticoagulant for VTE in critically ill patients with less adverse local complications at the injection site. The study was registered on www.clinicaltrials.gov Registration ID: NCT02795065. Registered June 8, 2016.

Palliative Care Utilization in Nontraumatic Intracerebral Hemorrhage in the United States*
Objectives: Palliative care is now recognized as an essential component of comprehensive care in serious illness that interferes with quality of life. We explored utilization of palliative care in spontaneous intracerebral hemorrhage at a population level using a large national database. Design: Population based cross-sectional study. Setting: Inpatient hospital admissions from the Nationwide Inpatient Sample. Patients: A total of 311,217 patients with intracerebral hemorrhage. Interventions: Palliative care use. Measurements and Main Results: Intracerebral hemorrhage patients with and without palliative care were ide...

The Range of Cardiogenic Shock Survival by Clinical Stage: Data From the Critical Care Cardiology Trials Network Registry
OBJECTIVES: Cardiogenic shock presents with variable severity. Categorizing cardiogenic shock into clinical stages may improve risk stratification and patient selection for therapies. We sought to determine whether a structured implementation of the 2019 Society for Cardiovascular Angiography and Interventions clinical cardiogenic shock staging criteria that is ascertainable in clinical registries discriminates mortality in a contemporary population with or at-risk for cardiogenic shock. DESIGN: We developed a pragmatic application of the Society for Cardiovascular Angiography and Interventions cardiogenic shock...

Comparative study between elderly and younger patients with acute coronary syndrome
Conclusion Dyslipidemia, hypertension and diabetes were the most frequent risk factors for CAD in elderly, while smoking was the most frequent risk factor in younger. Mortality was more frequent in elderly than younger. Complications were more frequent in elderly than younger. A predictor of Heart failure was an increase in both age and TIMI score.

Acute arterial thrombosis associated with inadvertent high dose of tranexamic acid
Surjya Prasad Upadhyay, Piyush N Mallick, Manish Jagia, Raj Kumar A SinghIndian Journal of Critical Care Medicine 2013 17(4):237-239Tranexamic acid (TA) act as anti-fibrinolytic agent and is widely used to limit bleeding in clinical practice. Tranexemic acid bind with plasminogen and prevent its conversion to plasmin, which limits the fibrinolytic pathway, so there is a theoretical risk of increasing thrombosis with high or prolonged therapy with TA. We encountered a case of acute arterial thrombosis following inadvertent administration of high dose of TA. A 27-years-old male with no other co-morbidity was ordered intraven...

Takotsubo cardiomyopathy in a patient with pituitary adenoma and secondary adrenal insufficiency
We describe a case of Takotsubo cardiomyopathy in a case of pituitary macroadenoma in acute adrenal crisis. A 48-year-old man presented with acute onset altered sensorium, vomiting, and gasping. On admission, he was unresponsive and hemodynamically unstable. He was intubated and ventilated and resuscitated with fluids and inotropes. The biochemical evaluation revealed hyponatremia, hyperkalemia, and hypocortisolism. Hyponatremia was corrected with 3% hypertonic saline. Contrast enhanced computed tomography (CT) scan of the brain revealed a sellar-suprasellar mass with hypothalamic extension with no evidence of pitui...

Arrive: A retrospective registry of Indian patients with venous thromboembolism
Conclusion: Acute DVT alone was responsible for the substantial burden of VTE in Indian patients. Bleeding was not the limiting factor for anticoagulant treatment in most patients.

Temporary left ventricular pacing: A desperate life-saving measure in emergency situation
Ajaz Ahamad Lone, Mohd Iqbal Dar, Fayaz Ahamad Rather, Mohd Sultan Alai, Imran Hafiz, Jahangir Rashid BeighIndian Journal of Critical Care Medicine 2017 21(8):531-533Transcutaneous or transvenous pacing of the right ventricle is performed as a routine practice for patients received with symptomatic bradycardia or complete heart block with relative ease in cath lab. However, more and more patients are received with multiple comorbidities, critical condition, and difficult vascular access. In this article, we describe a patient with difficult venous access with tricuspid regurgitation and displaced the right ventricular pace...

Prognostic value of T peak-to-end interval for risk stratification after acute myocardial infarction
Conclusion TpTe was significantly and independently associated with increased odds of SCD and is linked to deterioration of Lv functions and myocardial aneurysms. It's highly correlated to presence of LAS and associate with severity of coronary lesions. Patients with prolonged TpTe intervals and dispersions were likely to develop fatal arrhythmias.

Primary aorto-esophageal fistula: Great masquerader of esophageal variceal bleeding
Laxmi Kokatnur, Mohan RudrappaIndian Journal of Critical Care Medicine 2015 19(2):119-121Aorto-esophageal fistula is a rare cause of upper gastrointestinal bleeding. Thoracic aneurysm, the most common cause of this condition, will slowly increase over time and can erode the wall of the aorta creating a fistula and leading to torrential bleeding. High clinical suspicion is required for timely diagnosis as common investigations routinely done for gastrointestinal (GI) bleeding, including esophagogastroduodenoscopy, fails to detect most cases. The classical triad of midthoracic pain, herald bleeding and fatal hematemesis desc...
